What you will need to bring to the exam:

We bill insurance for PPO plans contracted with After Hour Pediatrics.
We ask for payment in full for HMO plans and out-of-network plans.

-Physical Exam Ages 8 to 11:  $175 
-Physical Exam Ages 12 to 17:  $190
-Urinalysis $10
-Hemoglobin $20 
-Vision $25
-Tuberculosis Skin Test $30
-Tdap Vaccine $70
